Ring Inland revenue and ask for your last 5 years employment history and they will post it to you that's what I'v done and that will be on there everything you'll need employment history,Tax code, NI number, home address etc and will have start and finishing dates.

I asked for that last week but they said the estimated arrival date is 5th July, so not looking too promising. When did you get yours?
